Job Description

1. POSITIONS VACANT: Consultant- Care Navigator, International Not-for-Profit Organization, Sihora (1 Vacancy) and Patan (1 Vacancy), Jabalpur, Madhya Pradesh.

 

2. ORGANISATIONAL BACKGROUND:

 

Our client is a respected international not-for-profit, that works in public health, to improve the health of disadvantaged communities, across the globe.

3. JOB DESCRIPTION/ RESPONSIBILITIES:

 

The Public Health Consultant- Care Navigator will provide field-level support to the public health system under the iWin project to strengthen client-centered, continuous, and coordinated maternal and newborn health care. The position focuses on supporting pregnant women and newborns, especially high-risk cases, by facilitating timely access to quality MNH services and reducing barriers to care.

The Care Navigator will work closely with ANMs, staff nurses, Medical Officers, and program staff to ensure effective service delivery, follow-up, and referral within existing government health systems. She/he will be responsible for the following tasks:

Key Job Responsibilities: 

 

(1) Support ANMs in improving early registrations by close coordination with ASHAs and from the eligible couple list;

(2) Facilitate follow ups and tracking to ensure four ANC visits happen for all registered women;
(3) Facilitate the HRP tracking and data validation mechanisms in the field;
(4) Facilitate and track PMSMA visit for all registered women;
(5) Track status of essential investigations for all registered women;
(6) Ensure adequate recording and reporting of ANC checkups and also in ANMOL application at both ANM level and facility level;
(7) Escalate issues related to IT support consultants at district and do regular follow up;
(8) Inform Jhpiego district team on ANMOL Sakhi logistics/device issues and ensure timely resolution;
(9) Support ANMs in batching and matching of PW with the appropriate delivery point;
(10) Conducting home visit to ANC/PNC mothers within the block to initiate beneficiary centered feedback mechanism. (especially focus on HRPs and follow up with their services, also in home delivered women too for a cause analysis);
(11) Coordinate with SUMAN helpdesk and provide necessary support;

(12) Support ANMs and ASHAs in planning PNC/HBNC visits, including perinatal mental health follow-up;

(13) Promote proper use of ANMOL Sakhi devices and help resolve field-level issues;

(14) Feedback to be shared with block and district officials on the basis of field visits. Daily updates of the work need to be submitted and shared with project officer. Also plan the activities on week to week basis and review if needed to meet the timelines & coverage across designated facilities;

(15) Any other responsibility assigned by Supervisor;


4. QUALIFICATIONS, EXPERIENCE AND COMPETENCIES:

 

Qualifications and Experience
(1) Applicant must have 12th pass, active, smartphone user, with zero addiction;

(2) At least 0-1 years of experience of working with public health programs preferred;

 

Skills and Competencies:
(1) Excellent verbal communications skills;
(2) Acceptable to local community, system and FLWs;

(3) Active Smartphone user;
(4) Have own bike and willingness to travel locally for program activities;


5. CONSULTANCY FEE: The monthly consultancy fee budgeted for the position is INR 22,000/- and overall CTC for 6 months is INR 132,000/-.

8. NATURE OF CONTRACT:

 

The selected candidate shall be engaged as a Consultant on the rolls of SAMS, a third-party payrolling agency, for a full-time consultancy engagement initially offered for a period of six months. The contract is extendable based on performance and project requirements. It is explicitly clarified that this consultancy engagement does not constitute a promise or guarantee of future employment, with SAMS, or our client, or any other organization associated with the project.
