Purpose
This Response Template has been designed to ensure a standardized and transparent evaluation process for all bidders. By providing your responses in this structured format, you enable our evaluation committee to assess all proposals fairly and consistently against the defined technical and financial criteria. Complete and accurate responses to all questions will help us understand your capabilities, experience, and proposed solution, ultimately allowing us to identify the most suitable partner for this critical public health initiative. Your thorough completion of this template is essential for your proposal to be properly evaluated. A few points for your consideration:

• The tender process will be conducted to ensure that tenders are evaluated fairly to ascertain the most economically advantageous tender (MEAT) criteria.
• Your response will be evaluated under the following headings, based on a weighting of 70% for technical evaluation and 30% for financial evaluation.
• Responses to all questions are mandatory unless otherwise stated in the RFP documents. Kindly input “NA” in case the question is not applicable to you. DO NOT LEAVE IT BLANK.
