REQUEST FOR QUOTATION (RFQ)

Procurement of Books for establishing Library

1.Background

MAMTA Health Institute for Mother and Child (MAMTA-HIMC), invites quotations from eligible and experienced vendors for the procurement and supply of books for establishing block level library in Laksar Block, District Haridwar, Uttarakhand.

2. Scope of Supply

The selected vendor shall be responsible for supplying books as per the specifications and quantities mentioned in the attached Annexure

The scope includes:

• Supply of books in accordance with the approved titles, and publishers.
• Ensuring all books are new, unused and in good physical condition.
• Ensuring books are authentic editions and free from printing or binding defects.
• Delivery of books at the designated location(s) in Laksar Block, Haridwar.
• Replacement of damaged, defective or incorrect books at no additional cost.

3. Eligibility Criteria

Interested vendors should:

• Have relevant experience in supplying educational books to schools or institutions.
• Be able to supply the required titles and quantities within the stipulated timeline.
• Submit a valid GST registration certificate, wherever applicable.
• Submit PAN and other applicable business registration documents.
• Provide references or details of previous similar assignments, if available.

4. Quotation Submission Requirements

The quotation should include:

• Vendor/firm name, address and contact details.
• GSTIN and PAN details.
• Item-wise unit price and total price.
• Applicable GST and other charges, if any.
• Transportation and delivery charges, clearly specified.
• Expected delivery timeline.
• Quotation validity period (minimum 30 days).
• Payment terms and conditions.

5. Terms and Conditions

• The quotation must be submitted on the vendor’s official letterhead, duly signed and stamped.
• Prices quoted should be inclusive of all applicable costs, including packaging, transportation and delivery, unless explicitly specified separately.
• The supplied books must strictly conform to the approved list of titles and specifications.
• The selected vendor shall complete delivery within the mutually agreed timeline.
• MAMTA-HIMC reserves the right to accept or reject any or all quotations without assigning any reason.
• The purchase order will be issued to the selected vendor after approval by the competent authority.
• Payment shall be made after successful delivery, verification and submission of the invoice and other required documents, as per the organization’s financial procedures.
• Any discrepancy, damage or shortfall in supply must be rectified by the vendor at no additional cost.

6. Submission of Quotation

Interested vendors are requested to submit their quotations on or before 18^th November through rfp@mamtahimc.in
